I frequently use the book module and export JPG files (It has Blurb, PDF *and* JPEG options!) . I then run a Photoshop batch process to crop my JPG files to whatever size I want. In my case I create a large square book and choose page templates that leave a large margin top and bottom so that my batch process crops to about A4 without losing any of my page content. I use autofill fill full bleed in my favorite print company to instantly fill these jpgs into my new book.
